Payment is due at the time services are rendered. Payments may be made using active and accepted insurance benefits or by self-pay.
If you have private insurance and would like to submit for potential out-of-network reimbursement, Ironclad Human Performance can provide you with a monthly superbill upon request. Reimbursement is not guaranteed and is subject to the terms of the patient's insurance plan.

Follow-Up Visits: $180
New patient evaluations are approximately 60 minutes and will establish a plan of care and targeted treatment goals. Concussion rehabilitation is typically a short rehabilitation course. Follow-up visits will last approximately 30 minutes.
